What is Support at Home?
Support at Home, delivered through Home Care Packages (HCPs), is an Australian Government program that provides funding for older Australians who need coordinated care services to remain living at home.
Home Care Packages are allocated based on your assessed care needs, ranging from Level 1 (basic care needs) to Level 4 (high care needs). The funding can be used flexibly to meet your individual needs and goals.
As an approved Home Care Provider, Southern Care Staff helps you access your package and works with you to create a personalised care plan that maximises your funding and supports your independence.

Home Care Package Funding Levels
Home Care Packages are available at four levels, each providing a different annual subsidy based on your assessed care needs.
Basic Care Needs
$10,633/year
For people with basic care needs who require a small amount of help to continue living independently.
Low Care Needs
$18,713/year
For people with low-level care needs who require more regular assistance with daily tasks.
Intermediate Care Needs
$40,743/year
For people with intermediate care needs requiring a variety of personal and clinical care services.
High Care Needs
$61,771/year
For people with high-level care needs requiring ongoing and complex support services.

Services Available Through Your Package
Your Home Care Package can fund a wide range of services to support your independence and wellbeing.
Personal Care
Assistance with showering, dressing, grooming, and other personal hygiene needs with dignity and respect.
Domestic Assistance
Help with household tasks including cleaning, laundry, changing bed linen, and general home maintenance.
Meal Preparation
Nutritious meal preparation considering dietary requirements, preferences, and cultural needs.
Transport Services
Accompanied transport to medical appointments, shopping, social activities, and community events.
Social Support
Companionship, social outings, and engagement in community activities to reduce isolation.
Respite Care
Short-term relief for primary carers, ensuring loved ones receive quality care while you take a break.
How to Access a Home Care Package
Follow these steps to access government-funded home care support.
Contact My Aged Care
Call My Aged Care on 1800 200 422 to register and request an assessment for a Home Care Package.
ACAT Assessment
An Aged Care Assessment Team will assess your care needs and determine your package level.
Receive Approval
Once approved, you'll receive a letter confirming your package level and referral code.
Choose Your Provider
Select Southern Care Staff as your preferred provider. We'll guide you through the process.
Develop Care Plan
We work with you to create a personalised care plan that meets your needs and goals.
Begin Services
